Public Health Institute of Metropolitan Chicago (PHIMC) seeks proposals from eligible organizations to provide high quality, high impact HIV prevention and linkage to care services for individuals at highest risk for transmitting or acquiring HIV.

This NOFO is issued by PHIMC as the Lead Organization on behalf of Illinois Department of Public Health (IDPH), HIV Section. This NOFO seeks proposals from organizations to provide HIV prevention services within Illinois Region 8 (Suburban Cook County, excluding City of Chicago) in the State Fiscal Year 2027, July 1, 2026- June 30, 2027. The Contract Period will begin upon Notice of Award through the end of the Fiscal Year. 

The purpose of this grant is to offer HIV Prevention Services through the following strategies:

• Expanding available HIV prevention services
• Enhancing HIV prevention service quality and comprehensiveness
• Leveraging resources through collaboration and linkage

Regional HIV Prevention awards issued under this NOFO will be composed of the following funding categories:

Direct Service

• Risk-Based HIV Testing and Referral Services
• Integrated STI Testing – GCCT, HCV, Syphilis
• Risk Reduction – Group Level and Individual Level Interventions
• Surveillance Based Services – Linkage to care and supportive services for individuals living with HIV
• Monthly Condom Distribution

Capacity Building

• Black/Latiné MSM Recruitment
• Black/Latiné MSM Peer Hiring and Training

This grant utilizes a Cost Budget Model of Reimbursement.

Key Dates

Eligibility

Only organizations based within Illinois are eligible to receive HIV Prevention grant funds. These organizations may be local health departments or not-for-profit private community-based organizations, including volunteer or religious organizations which effectively engage prioritized risk populations including men who have sex with men, transgender individuals, heterosexual black/latiné women and men, and people who inject drugs. Funded organizations may not be 501(c)(4) organizations or organizations whose primary mission is to engage in Illinois or federal lobbying activities.

Pre-Proposal Conference

PHIMC strongly encourages all interested applicants to attend the pre-proposal conference on Tuesday, September 8th, 2026, 1:00 – 3:00pm. This webinar will provide you with detailed instructions and technical assistance for completing the proposal and submitting supporting documents.

Application Submission

All applications and required documents must be submitted electronically to NOFO@phimc.org by Thursday, October 1, 2026 at 5:00 p.m. The applicant is solely responsible for submitting a complete and timely application. Hard copies of the application will not be accepted. Applications received after the deadline date and time will not be considered for this opportunity. Only organizations that have submitted the Intent to Apply Form by 9/17/2026 at 5:00 p.m. CDT will be considered for this grant.
