The Transgender and Gender Diverse (TGD) Wellness and Equity Program provides comprehensive, lifesaving, and medically necessary gender-affirming care to transgender, gender diverse, and LGBTQ+ people throughout the state of Illinois. Each region across the state has an agency with a Healthcare Navigation Specialist (HNS) who can help people find healthcare and social services that they need. PHIMC serves as the lead grantee for this program, which is funded by the Illinois Department of Human Services.
