Request for Applications for Substance Use Disorder (SUD) Allies Host Sites

Public Health Institute of Metropolitan Chicago (PHIMC) will release a Request for Applications (RFA) for Substance Use Disorder (SUD) Allies host sites the week of July 30, 2018.
SUD Allies supports prevention, treatment, and recovery agencies throughout Chicago and suburban Cook County that serve individuals and families impacted by substance use disorders. SUD Allies is a cohort of twelve AmeriCorps members that will serve in up to twelve eligible organizations for eleven months supporting projects that align with SUD Allies goals, objectives, and/or strategies. These AmeriCorps members engage clients, youth, and providers through a variety of comprehensive services. Funding for this AmeriCorps program is made available by the Serve Illinois Commission on Volunteerism and Community Service, a part of the Illinois Department of Public Health, in partnership with the Corporation for National and Community Service.
PHIMC seeks applications from organizations that provide SUD services and/or prevention to host an AmeriCorps member for a period of eleven months. Host site applicants may request more than one AmeriCorps member. The RFA is limited to positions that create opportunities for AmeriCorps members to provide one or more of the following direct services to populations in SUD treatment and/or recovery or at high risk for SUD:
- Case Management
- Health Insurance Enrollment Assistance
- Anti-Stigma Education
- Prevention Education

Eligibility Information:
Applicants must:
- Be a 501(c)(3) public or private nonprofit organization; institution of higher education; government entity within a state or territory; Indian Tribe; or a partnership/consortium
- Provide substance use disorder services and prevention within the City of Chicago or Suburban Cook County
- Provide a cash contribution in the amount of $13,500 per member
