2025 Community Reentry Champion Award


The Public Health Institute of Metropolitan Chicago (PHIMC) is proud to recognize and celebrate exemplary organizations or individuals with lived experience in Illinois who have made significant contributions to the reentry space and for/with individuals who are justice-impacted, with a particular emphasis on innovative programs, advocacy, research, direct service, and HIV prevention efforts. The Community Reentry Champion Award aims to honor those who work tirelessly, not only toward a more equitable and supportive reentry process but also towards the eradication of HIV/AIDS among individuals returning to Illinois communities following incarceration.

Nominations can be submitted here by February 21, 2025

Nominations should include a clear and detailed description of the nominee’s work to support people reentering communities and efforts to prevent HIV. Please refer to the submission guidelines on the nomination form for more information on the award criteria, or download the award criteria here.

Winners will be notified by February 28, 2025, and awards will be presented during PHIMC’s 2025 Illinois Reentry Conference on Thursday, March 13 2025.
