DESCRIPTION:PHIMC hosts a Corrections Discharge Planning Training (CDPT) to enhance provider capacity to address the complex needs of formerly incarcerated individuals. This one-day workshop consists of four modules facilitated by key thought leaders and professionals inclusive of individuals with lived experience in both the corrections and public health fields.
