DESCRIPTION:Join PHIMC and other thought leaders for a free one-day training designed to educate and inform providers in both community-based and healthcare settings\, on medical and non-medical discharge planning for formerly incarcerated individuals.This one-day training consists of four 75-minute modules on the topics listed below. Modules are facilitated by thought leaders and professionals inclusive of individuals with lived experience in both the corrections and public health fields. This training covers: Overview of the Corrections System and Reentry ProcessTrauma-Informed Care for the Corrections PopulationBasic Needs and Barriers for Formerly Incarcerated IndividualsHealth-Related Topics that include HIV\, Substance Use\, and Mental HealthContinuing Education Units are offered. Note requirements below. –> Register here <– In order to receive Continuing Education Units (CE) for participation in this training\, you must:1. Register for the conference in advance of the scheduled date of the conference.2. Use the exact same name and email address you used when registering for the conference when you enter and attend the training.3. Remain and be present during the entire duration of the training. 4. Complete both CDPT pre and post tests the day of the training.5. Complete the overall CDPT evaluation day of training no later than two days after the training.Please understand that if you do not comply with all 5 requirements listed above\, a CE will not be issued.
